Dynamo Gymnastics Tuition & Policies

 

About Tuition :
· Tuition is due on or before your first class. Payments must be made in full. Amounts will be prorated if your child starts after the session start date.
· Tuition cannot be paid for on a weekly basis. In some cases, payment arrangements can be made. Approval of this must be given by Director and arrangements must be made prior to the first class.
· Accounts will not be prorated or refunded for missed days. No refunds will be given for classes missed including scheduled holidays. No refunds will be given but a credit will be credited to the student’s account to be used for birthday parties, playzone visits or purchases in the proshop.
 

About Policies :
· No spot is guaranteed until payment is made.
· If after the first class a payment has not been received, your child will be removed from the class. Please make arrangements in advance if you know your child will miss the first class and payment has not been made.
· Students are allowed one make-up class per 10-week session. Make-ups must be within the 10-week session in which the student is absent. We do not roll over missed classes to the next session.

 

REGISTRATION FEE

All Students will be charged a $35 registration/anniversary fee. This fee is good for one year or until you drop out of the program.

Because of our strict student to teacher ratio, students are allowed one make-up per 10 week session. Make-ups must be within the 10 week session in which the student was absent. WHAT TO WEAR

Boys or Girls may wear tucked in t-shirts and shorts OR Leotards for Girls. NO chewing gum or dangling jewelry. Hair should be pulled neatly and securely away from the face so that is stays up for the entire workout. Girls should not wear bows or other large hair ornaments that may cause discomfort durring activity. Personal items should be left in cubby holes. Jewelry should not be worn during classes. Please leave jewelry articles at home. Dynamo Gymnastics is not responsible for ANY items that may be lost or stolen. Be sure your student’s personal items are marked with their name.

 

WAIVER

Having been informed of the activities to be conducted by Dynamo Gymnastics and Playzone program, I, as a parent or legal guardian for the participant named on the reverse side of the registration form, consent to the student’s participation in any and all activities of the program. I assume all risks and hazards of the program, including, but not limited to, transportation to and from these activities. I further release, waive, and forever discharge any and all rights and claims against Dynamo Gymnastics and Playzone, its directors, owners, instructors, and employees, holding them harmless for any illness or injury of the participant occurring during the program.

 

As the parent or guardian of the participant, I understand that is my responsibility to be in the Playzone at all times to supervise the participant. I understand that it is my option to consult a physician for assurance of proper health and have been encouraged to do so by Dynamo Gymnastics and Playzone. Furthermore, I hereby authorize the directors of Dynamo Gymnastics and Playzone to act for me according to their best judgment in any emergency requiring medical attention. I know of no mental or physical problems which affect the participant’s ability to participate safely in the Dynamo Gymnastics and Playzone program. I understand that should I have any questions or concerns regarding the Dynamo Gymnastics and Playzone program, I may contact a director at 405-728-0500.

WAIVER/RELEASE FOR COMMUNICABLE DISEASES INCLUDING COVID-19

ASSUMPTION OF RISK / WAIVER OF LIABILITY / INDEMNIFICATION AGREEMENT

In consideration of being allowed to participate on behalf of the Dynamo Gymnastics athletic program and related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. Participation includes possible exposure to and illness from infectious diseases including but not limited to MRSA, influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist; and,

2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

3. I willingly agree to comply with the stated and customary terms and conditions for participation as regards protection against infectious diseases. If, however, I observe and any unusual or significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and,

4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Dynamo Gymnastics their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event (“RELEASEES”), WITH RESPECT TO ANY AND ALL ILLNESS, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

 

FOR PARTICIPANTS OF MINORITY AGE (UNDER AGE 18 AT THE TIME OF REGISTRATION)

This is to certify that I, as parent/guardian, with legal responsibility for this participant, have read and explained the provisions in this waiver/release to my child/ward including the risks of presence and participation and his/her personal responsibilities for adhering to the rules and regulations for protection against communicable diseases. Furthermore, my child/ward understands and accepts these risks and responsibilities. I, for myself, my spouse, and child/ward do consent and agree to his/her release provided above for all the Releasees and myself, my spouse, and child/ward do release and agree to indemnify and hold harmless the Releasees for any and all liabilities incident to my minor child’s/ward’s presence or participation in these activities as provided above, EVEN IF ARISING FROM THEIR NEGLIGENCE, to the fullest extent provided by law.

 

BILLING AUTHORIZATION

I represent and warrant that if I am purchasing something from this facility or from Merchants that (i) any credit or bank account draft (ACH Draft) information I supply is true and complete, (ii) charges incurred by me will be honored by my credit card company or financial institution, and (iii) I will pay the charges incurred by me at the posted prices, including any applicable taxes, fees, and penalties.

 

I hereby authorize (if online payment is made or autopay information is provided) this facility to charge my bank, or credit card account. I understand that a 30 day written notice is required to terminate billing and I am responsible for payment whether or not my student attends classes until I notify this facility in writing to drop my student from classes.

 

Should I dispute a charge through my financial institution this will constitute a breach of contract possibly resulting in, but not limited to, penalties, additional fees, collection, legal action, and/or termination of any and/or all current and future services.

WEATHER POLICY 

If Putnam City Schools close due to inclement weather Dynamo Gymnastics and Playzone will also close.
